    Transportation Designation Form

  • Dear KMS Parents,

    Ensuring accurate daily transportation for your child is one of our top priorities. However, it is a task that requires your assistance. Please fill out the following electronic form as completely as possible. The more information that is provided, the better job we can do to serve your students!

    REMINDERS:

    • If you need to make changes to this form AFTER school begins, a note must be sent in to your child's teacher. This note will be forwarded to the KMS Office and allows the teacher to have the most up-to-date information.
    • ALL notes must have your child’s full name, and in the case of parent pickup, also the full name of the person picking up the student. In the case of a bus stop change, we will need the full address of new stop.
    • IMPORTANT: ANY NOTES FROM THE PREVIOUS YEAR REGARDING BUSSING OR PARENT PICK UP ARE NOT VALID FOR THE CURRENT SCHOOL YEAR!

  • 3A. Bus Student Standard Pickup/Dropoff - Transportation Details

  • 3B. Please list the FULL NAMES of all individuals, including PARENTS, who are to be placed on your child's parent/guardian pick-up list. By including an individual's name on this list, you are giving permission for the individuals listed to PICK UP your child from the school AT ANY TIME.

    NOTE: a valid ID will need to be shown by the listed individual for EVERY PICKUP.
